Please note that there are two liability forms that must be completed for your student to attend camp: one for Camp Connect and one for Camp Copass. Both forms are required. You can find the links to the forms at the bottom of this page.

All individuals attending Camp Connect must complete a liability form. Please fill out a form for each student and return it to your student pastor. If you are unable to print the form, please contact your pastor or the camp secretary for a copy.
